APPLICATION FORM FOR DATA OWNERS TO EXERCISE THEIR RIGHTS UNDER THE LAW NO. 6698 ON THE PROTECTION OF PERSONAL DATA
Within the scope of Article 11 of the Law No. 6698 on the Protection of Personal Data (“KVKK”), which entered into force on 6 April 2016, certain rights have been granted to personal data owners. Please fill out the following application form clearly and completely in order to communicate your requests regarding your personal data, which is processed by MİDAS HEDİYELİK EŞYA SANAYİ VE TİCARET ANONİM ŞİRKETİ having its registered office at Yalçın Koreş Cad.No.18 Bahçelievler/İstanbul and being registered under MERSIS No. 0621002146400017 in the capacity of data controller, containing your wet signature and deliver it to our address given above by hand or by registered mail. We will respond to your application as soon as possible, no later than 30 days after we receive it.
In case the information and documents you have sent us are incomplete or your request requires an additional cost, we will contact you to ask for your confirmation to proceed with the relevant transactions.
The right to apply to our company with this form prepared in accordance with Article 11 of KVKK belongs directly to the personal data owner. For applications to be made on behalf of third parties, a power of attorney indicating that you are authorized to represent the person concerned must be present.
